It is important your Patient Information stored on our Computer System is current and is updated as your details change.

In the case of an emergency, we need to have details of  Next of Kin and Emergency contact details.

Your Medical Records

Each patient has their own Medical Record.  This record contains the Doctors notes of your history and previous medication.

These are the Doctors clinical notes.  All consultations with your Doctor are private and confidential and cannot be revealed to any other persons without written consent, a court order or subpoena.

Patient Feedback

From time to time this practice invites patients to complete a questionnaire on their view of the clinic and how it could be improved.  These surveys are completely confidential and help us to improve our services.  If you are unhappy with any aspect of the care you receive from this clinic we are very keen to know about it.  Please feel free to talk to your Doctor or the Receptionist about any problems you have with the service we provide.

This Clinic is committed to providing comprehensive general practice care to all individuals and their families in this community.  All patient consultations and medical records are kept strictly confidential.  We believe that problems are best dealt within the clinic.  Indeed, we want to know if you are concerned about any aspect of our service.  However, if you feel there is a problem you wish to take up outside you may prefer to contact the Victorian Government Centre for handling complaints.
